Your invitation to join the portal will arrive by SMS text message and/or email. (SMS text messages will come from 'CCCHOSPITAL'. Emails will come from 'no-reply-uk@patientsknowbest.com'.)
Please make sure we have the correct contact details for you when you check in. Don’t forget to tell us about any changes. You will also need to let your GP practice know as they hold your main NHS record.
